1. A person who is not more than sixty-five years of age on the last date fixed for submission of applications for the post may apply.
2. The candidate having a conflict of interest with the above-said post will not be eligible i.e.

• The candidate should not involve in organ transplantation.
• The candidate should not own the Clinic or Hospital where Organ transplantation is performed.
• The candidate should not be a member of the Monitoring Authority of PHOTA.
• The final appointment will be subject to the verification of educational degrees & experience certificates (any expenditure in this regard will be borne by the candidate/ employee).

• Applicants are required to submit application/dossiers on the prescribed proforma which could be downloaded from the website www.phota.punjab.gov.pk, to the Dairy Section at the Office of Director General/Administrator of PHOTA, 39-Shadman-1, near Shadrnan Market, Lahore, along with a bank draft of Rs. 2000/- in favor of Director General / Administrator PHOTA Lahore, along with attested copies of the following mandatory documents:

• All the educational Degrees/Diplomas/Certificates.
• Valid PMDC/PMC registration Certificate.
• Provisional Certificate/Result Cards of all professional examinations

• Domicile.
• 3-Fresh passport size colored photographs.
• PMC/PMDC/HEC recognized research work / Publications (if any).
• The applicants having a foreign degree must have PMC/HEC equivalence certificate.
